Consequências biopsicosociais da guerra... Gaza orphans in the spotlight

GAZA CITY, 1 April 2009 (IRIN) - An estimated 1,346 children were left without one or more of their parents as a result of the recent 23-day Israeli assault on Gaza, according to Islamic Relief in Gaza.

An orphan is defined by Islamic Relief as a child under 18 who has lost the parent considered the head of the household, most often the father, according to a child welfare programme manager for Islamic Relief Mahmoud in Gaza, Abudraz. The official UNICEF definition of an orphan is a child who has lost one or both parents.

There are currently about 5,200 orphans under Islamic Relief's definition in Gaza.

"There has been a dramatic increase in [the number of] requests for sponsorship [of such children] in Gaza," Abudraz said.

Over 500 such requests have been made recently and Islamic Relief has responded positively to 200 of them. Eligible orphans must be under 14 and in the care of a low-income family (earning 1,000 shekels - US$240 - per month or less) with more than four members.

Extended families in Gaza cannot support the orphans in the long term, said Abudraz.

Food, cash aid

Two-year-old twins Ahmed and Samia (their guardian declined to give their family name) from the al-Tufa area of Gaza City lost their father on 14 January. Their uncle, Mahmoud, is now their primary caretaker after their mother began working as a janitor for 400 shekels ($100) per month.

Mahmoud, a former police officer, is now disabled after having been wounded by an Israeli bomb. He and his wife already have seven children, two of whom are also disabled.

"I care for the twins as if they are my own, but I only earn 1,400 shekels (about $336) a month [from a disability allowance]," said Mahmoud, standing outside his home, which was shelled during the offensive.

The twins receive food coupons and 200 shekels (about $50) each per month from Islamic Relief.

About six children lost both of their parents and about 1,300 children lost one of their parents during the recent conflict, according to Baker al-Torukmane, an attorney for the Palestinian Centre for Democracy and Conflict Resolution (PCDCR). The Centre provides psychological and legal counselling for orphans and surviving parents.

Al-Salah Islamic Society in Gaza is providing emergency food baskets and 100-200 shekels ($50-100) per month to several hundred of the new orphans. Their website [www.alsalah.org] requests sponsors for the orphans. Other Palestinian NGOs are also providing emergency cash assistance and food packages.

The UN Children's Fund (UNICEF) is currently conducting an assessment of the number of orphaned children in Gaza.

This handbook contains a professional-level overview of the current state of research on anxiety and fear. The text covers a wide range of material that includes genetics, neuroanatomy, pharmacology, behavioral observations, and clinical applications.
In the initial chapters, there is considerable effort made to define anxiety, fear, and panic as unique responses with their own neuroanatomical components. Clinical discussions focus on the five most common anxiety disorders: panic disorder, obsessive-compulsive disorder, social anxiety disorder, posttraumatic stress disorder, and generalized anxiety disorder. They cover a wide range of relevant topics such as genetics, pharmacology, brain imaging, hormones, and symptomatology.
This text is oriented toward basic research, so the clinical information seems disconnected from the previous chapters. The handbook is a lengthy and thorough review of the experimental, neurobiological, pharmacological, and clinical components of anxiety and fear. This text is best suited to professionals and advanced graduate students who research anxiety disorders. Professionals wishing to review the scientific bases of the anxiety disorders would do well to include this book in their personal libraries. The chapters of this book focus on key topics in positive psychology such as happiness and well-being, positive emotions, money and happiness, positive traits, and close relationships. Of course, not all topics that one may expect to be relevant to positive psychology are represented in depth in the present book. However, Baumgardner and Crothers have authored a strong, well-written text that is certain to make a valuable contribution.
Crafting a successful text for an emerging field such as positive psychology presents a number of challenges, many of which the authors have gracefully met, and one looks forward to the enhancements and expansions that will certainly accompany the revised editions of positive psychology’s next generation. SUDAN: Expulsions leave gaps in Three Areas, eastern region

NAIROBI, 31 March 2009 (IRIN) - NGO expulsions have left humanitarian gaps not only in Darfur, but also in eastern Sudan and the so-called Three Areas bordering on Southern Sudan, Abyei, Southern Kordofan and Blue Nile - volatile regions key to the success of a 2005 peace accord.

"The expulsions have left large parts of the Three Areas and eastern Sudan without humanitarian assistance or recovery and reintegration support," writes Sara Pantuliano, research fellow with the Humanitarian Policy Group of the Overseas Development Institute. "Unlike in Darfur, there is very little additional capacity beyond the expelled agencies to even attempt to fill these gaps."

Before its expulsion, Oxfam GB was working in Red Sea State, eastern Sudan. "We have been working with very remote, marginalised communities who have very little support from anywhere else," Alun McDonald, Oxfam GB regional media and communications officer for Horn, East and Central Africa, told IRIN. The region, which has high rates of poverty, malnutrition and illiteracy, often suffers regular floods and droughts.

"Last time the floods hit, many villages were submerged and thousands of people lost their homes, animals and farms... if the floods strike again this year, with nearly all the aid agencies expelled, communities will be extremely vulnerable," McDonald said. "The decision to expel us from eastern Sudan will affect the poorest people in the state."

Children at risk

Kassala and Red Sea states have the highest malnutrition rates in Sudan, according to ODI. The UN Children's Fund (UNICEF) estimates that the expulsions will leave more than 100,000 vulnerable children in northern Sudan without support.

Access to health services has also been reduced in Southern Kordofan. According to the UN World Health Organization (WHO), at least 30 percent of the state's health facilities remain without direct implementing partner support, and may suspend services in routine immunisation, nutrition and feeding programmes following the expulsions.

"...The expulsion means we can no longer support 56 health clinics that we've helped to build or rehabilitate since the peace agreement in 2005. We can no longer provide these clinics with essential medicines, staff training or support for community health education initiatives," Kurt Tjossem, International Rescue Committee (IRC) regional director for the Horn and East Africa region, told IRIN.

Before its expulsion, the IRC was working in Kassala, Red Sea, Blue Nile and Southern Kordofan "... supporting... lifesaving medical care, water, sanitation and livelihoods for around 1.1 million people".

Now, the IRC has been forced to stop its water and sanitation programmes in these areas. Fewer than 40 percent of the population of Kassala and Red Sea states have access to safe drinking water.

"In former SPLM [Sudan Peoples' Liberation Movement, which governs Southern Sudan and is a partner in the national unity government, GNU] areas of both Southern Kordofan and Blue Nile, particularly Kaoda and Kurmuk, NGOs deliver most essential services," according to the ODI.

The expulsion of the Norwegian Refugee Council (NRC) and Save the Children UK also affected education and child protection projects. NRC had been active in Sudan since 2004 and was implementing education and school construction projects in Southern Kordofan.

Save the Children UK was working with community groups on children's rights, gender-based violence and HIV/AIDS. "This [the expulsion] has very worrying implications for the 50,000 children we are currently supporting in Khartoum and the north-east of the country," said Ken Caldwell, Save the Children UK's director of international operations.

Peace threat

According to ODI, "it may also be difficult for Sudanese NGOs from Northern Sudan to work in some of the more politicised camps in... former SPLM-controlled areas of Southern Kordofan and Blue Nile".

The expulsions are also "likely to have a direct negative impact on ongoing efforts to promote peace and stability, especially in Abyei and Southern Kordofan, both through the provision of 'peace dividends' and reconciliation activities", according to Pantuliano. "These areas are already unserved or underserved by the state GNU, and the withdrawal of services provided by departing NGOs will increase levels of frustration." There has been growing discontent in Southern Kordofan.

Abyei, Southern Kordofan, Blue Nile and the eastern region have only recently emerged from conflict and remain highly volatile, she said. Clashes between the Sudan Armed Forces and the Sudan People's Liberation Army (SPLA) in May 2008 displaced about 60,000 civilians in Southern Kordofan and Abyei.

Relations within the GNU could also be affected. "... The decision to expel the international NGOs may also increase tensions between the ruling National Congress Party and the SPLM, as it appears that the latter was not consulted on the decision," according to ODI. "The SPLM is seeking to contest the move in order to allow the organisations concerned to continue operating at least in former SPLM-controlled areas of Blue Nile and Southern Kordofan, as well as in Abyei.

"As with the SPLM, the Eastern Front was not consulted about the expulsions. The front, which is currently undergoing a political crisis, is likely to be further alienated from Eastern Sudan society, particularly youth, potentially fuelling fresh unrest."

TIMOR-LESTE: Abortion laws in spotlight

Abortion remains illegal in largely Catholic Timor-Liste
DILI, 18 March 2009 (IRIN) - A call for more lenient abortion legislation in this predominantly Catholic country is renewing friction between the Church and pro-abortion activists.

A working group convened by Fokupers (“Communication Forum for Women from the East”), a local NGO supported by others such as the Alola Foundation, has been pushing for a softening of abortion laws.

The issue was highlighted in Dili, the capital, at the second international Women for Peace Conference from 4 to 6 March.

Maria Barreto, programme manager for advocacy at Fokupers, told attendees that abortion should be decriminalised in certain situations.

“Abortion is one of the options that is appropriate when the mothers are victims of sexual violence. We are working to protect women. We should understand that we should give options to mothers based on their circumstances,” Barreto told IRIN.

Abortion is criminalised under a penal code dating back to the Indonesian occupation of 1975-1999. Fokupers is one of several NGOs pushing for the government to relax the law.

However, in early March, the Dili and Baucau diocese wrote to the Timor-Leste Council of Ministers, the political executive with the power to pass laws, requesting that abortion remain criminalised in all instances.

The council later discussed a new penal code, including the proposal to soften the law on abortion. A decision has yet to be made.

At the end of the conference, one of the recommendations put forward by the panel was that the new code should include three circumstances under which abortion is permissible: cases of incest, sexual abuse and if the mother or baby’s life is at risk.

However, the move is fiercely opposed by the Catholic Church. About 95 percent of Timor-Leste’s 1.1-million population are Catholic.

Sister Guilhermina Marçal of the Canossian Sisters Order in Dili told IRIN the solution should come from tackling fundamental problems, such as poverty, post-conflict trauma and unemployment. “Education is very important to transform people’s minds and moral values. We have a programme for visiting families and we hold gatherings of the youth members,” she said.

Illegal abortions

Barreto told the conference that incest occurred frequently in remote areas. Often parents and children slept in the same room, leaving little privacy and sometimes leading to sexual abuse and unwanted pregnancy.

In February, Charles Darwin University researcher Suzanne Belton conducted a study on unwanted pregnancy in Timor-Leste, concluding that the law was highly restrictive and that back-street abortions were common.

Indeed, conference coordinator Filomena Barros Dos Reis said: “There is a lot of gender-based violence in Timor-Leste. Domestic violence and incest are not openly discussed as in other countries. There are unwanted pregnancies as a result of sexual assaults.”

Susan Kendall, international mentor for Psychosocial Recovery and Development East Timor (PRADET), a local NGO, told IRIN, “Children are much more likely to be sexually assaulted by someone they know. Part of the problem is the shame that’s put on the parents. Sexual assault is not talked about.”

At the conference, Kendall said about one-third of the cases of domestic violence the group encountered were alcohol-related. The NGO has been running workshops to raise awareness of the dangers of alcohol abuse to reduce the incidents of domestic violence and sexual assault.

UN tracks rising violence against women in Gaza

GAZA CITY, 24 March 2009 (IRIN) - The UN Development Fund for Women (UNIFEM) in Gaza, local Palestinian NGOs and mental health professionals are reporting increased incidents of domestic violence and sexual assault against women in Gaza since the beginning of 2009.

An unpublished UNIFEM survey of male and female heads of 1,100 Gaza households conducted between 28 February and 3 March indicates there was an increase in violence against women during and after the 23-day war which ended on 18 January.

“According to our staff, and through clinical observation, there was increased violence against women and children during and after the war,” said public relations coordinator for the Gaza Community Mental Health Programme (GCMHP), Husam al-Nounou.

“We can attribute this to the fact that most people were exposed to traumatic incidents during the war, and one way people react to stress is to become violent.”

GCMHP, which runs six clinics and treats an estimated 2,000 mental health patients a year, carried out a post-war assessment, interviewing about 3,500 Gaza residents, said al-Nounou.

“This war was extremely harsh, people felt insecure, vulnerable and unable to protect themselves, their children and their families; when people were trapped at home this increased the stress and anxiety,” said al-Nounou.

Sahar (who wanted her family name omitted), aged 36, divorced her husband in February due to the physical and psychological abuse she endured leading up to and during the war.

“He beat me severely and I was fainting from the stress,” said Sahar. “He forced me to engage in sexual intercourse against my will.”

Sahar brings her two-year-old daughter to the Palestinian Centre for Democracy and Conflict Resolution (PCDCR) to visit the daughter’s father. The court ordered supervised visits after Sahar’s ex-husband and his brothers tried to take her daughter away by force.

“Before the war the centre was facilitating supervised visits for 30 families, but now it is doing this for 60 families,” said Bakr Turkmani, an attorney at the PCDCR.

“The number of divorce and separation cases has increased significantly since the war, and domestic violence played a role in the increase,” he said.

“My husband beat me and insulted me,” said another victim of domestic violence from Gaza City who preferred anonymity. Recently divorced, she also brings her nine-month old son to PCSCR for weekly supervised meetings with his father.

“If I do not accompany the victims to the police station, their reports of abuse are not accepted,” said Turkmani.

Director of the women’s unit at the leading Palestinian human rights organisation, the Palestinian Centre for Human Rights (PCHR), Muna As-Shawa, said the centre had received reports of increased domestic violence and sexual assault during and after the hostilities. The unit had counselled over 600 women.

“During and after the war women struggled to fulfil their roles as mothers, and care for their children without electricity and water, while under attack,” said As-Shawa, “and if the husband died, sometimes the father-in-law took the inheritance and tried to take custody of the children.”

PCHR is providing legal advice to widows.

The Women’s Affairs Centre (WAC) in Gaza said it had organised meetings with 200 women across Gaza after the war.

“Many women who never experienced violence at home, were beaten during the war,” WAC director Amal Siam told IRIN.

Scores of women who lost their husbands came to WAC seeking assistance after their fathers-in-law tried to take custody of their children, said Siam, adding that there had been an increased number of divorce cases during the hostilities.

According to UNIFEM, the results of the first UN inter-agency gender needs assessment are due in May.

Intervenção Psicossocial em Crise e Emergência em Portugal

Para começar, temos a informar que existe muita informação do que tem sido feito em Portugal nesta área nos arquivos do nosso blog. A nossa intenção não é de forma nenhuma reduzir o que tem sido feito em Portugal nesta área, mas sim, enquadrar no que é seguido internacionalmente.
Porque estamos nos grupos de trabalho europeus e internacionais e, porque seguimos regras de procedimentos mais que testados.

Salientamos a importância da intervenção seguindo regras explícitas porque estamos a trabalhar com seres humanos numa das alturas mais sensíveis das suas vidas, e, basta uma palavra errada como "tenha calma" para as consequências dos nossos actos serem muito nefastas.

Títulos importantes na intervenção em Crise e Emergência

Como devem saber, existem imensos manuais de intervenção em crise. Existe também em Portugal um boom na área em que muita gente começa a trabalhar sem saber bem o que está a fazer.

Com vista a minorar as consequências nefastas que uma má intervenção tem na pessoa afectada, sugiro a leitura dos seguintes documentos que irei também pôr em link do lado esquerdo:

IASC guidelines on mental health and psychosocial support in emergencies settings

É um esforço conjunto de vários organismos da ONU e ONG's Internacionais de desenvolver padrões mínimos de trabalho. Tem em vista organizar a confusão que tem sido a intervenção psicossocial em crise e emergência.
Este manual pretende que se tenha em atenção a população alvo da intervenção que efectuamos como parte activa da tomada de decisão e escolha do caminho de recuperação.
É especialmente importante para quem tem ilusões de ir para fora ajudar a fazer aquilo que acha que os outros precisam e não aquilo que eles realmente precisam.


European Policy Paper

Mais uma vez, desta a nível Europeu, a definição de padrões de intervenção. Tal como já devem estar a pensar, não é só em Portugal que se faz muita intervenção desadequada às reais necessidades. Devido às consequências das más intervenções, mais um manual básico para pensarem bem se realmente é isto que querem fazer.

Projecto de Investigação para a Liga de Bombeiros Portugueses

A Liga de Bombeiros Portugueses está a desenvolver bolsas de investigação.

BOLSAS DE INVESTIGAÇÃO
REGULAMENTO DE CANDIDATURA

Desde há muito que está identificada a necessidade de desenvolver no Sector de Bombeiros actividade de investigação, direccionada a áreas directamente associadas com as missões que lhe estão legalmente atribuídas.
Deste modo, torna-se indispensável estimular entre os diversos intervenientes das estruturas de Bombeiros, em especial entre os elementos inseridos nos corpos de bombeiros de qualquer natureza, a curiosidade pelo conhecimento e a investigação, com o objectivo de inovar as técnicas e os equipamentos no socorro prestado às populações.
A promoção de Bolsas para a Investigação constitui um contributo para a valorização dos Bombeiros, profissionais ou voluntários, bem como para a qualificação da protecção e socorro no nosso país.
O esforço que representará a atribuição anual destas Bolsas, terá como retorno o aumento da massa crítica de um sector carenciado da mesma.

2 – Requisitos gerais de candidatura
a) Os trabalhos de investigação a desenvolver serão propostos pelos interessados e deverão versar as matérias referidas no ponto 1;
b) Os candidatos deverão ter a nacionalidade portuguesa, estar obrigatoriamente inseridos no quadro de um corpo de bombeiros de qualquer natureza (profissional, misto, voluntário ou privativo) sedeado no território nacional, possuírem ou frequentarem uma licenciatura universitária;
c) As candidaturas deverão ser apresentadas através de formulário que pode ser obtido na Liga dos Bombeiros Portugueses, Rua Eduardo de Noronha n.º 5-7, 1700-151 Lisboa, ou solicitado por e-mail (sandra.teixeira@lbp.pt);
d) Os resultados das candidaturas serão comunicadas a todos os participantes no prazo de 30 dias após a recepção das mesmas;
e) A assinatura do protocolo com os candidatos seleccionados decorrerá nos 30 dias ubsequentes à data da comunicação sobre a respectiva selecção;
f) Para apreciação da validade técnica e científica das candidaturas será constituído um Júri de Selecção, presidido pelo Presidente do Conselho Executivo da Liga dos Bombeiros Portugueses, e integrado por um representante da Autoridade Nacional de Protecção Civil, um representante da Escola Nacional de Bombeiros, um representante do Montepio Geral, e por três personalidades de reconhecido mérito.
g) Os resultados dos trabalhos de investigação seleccionados ficarão propriedade da Liga dos Bombeiros Portugueses, que poderá dispor livremente dos mesmos para a sua publicação.

3 – Condições
a) Os trabalhos deverão ser redigidos em língua portuguesa;
b) Os projectos deverão ter sempre em consideração a legislação aplicável, portuguesa ou comunitária;
c) O período de realização dos trabalhos é de 6 meses, contados a partir da data de assinatura do protocolo referido na alínea e) do ponto 2 deste regulamento;
d) A dotação económica será de 3.000,00 € (três mil euros), entregue em fracções mensais ao longo do período de investigação, com excepção da última parte que será entregue com a recepção do trabalho;
e) Durante o período de duração dos trabalhos de investigação, deverá ser garantida total confidencialidade sobre os mesmos,
os quais não poderão ser divulgados nem ser objecto de publicação prévia;
f) Mensalmente os bolseiros deverão apresentar um relatório de progresso dos trabalhos em curso;
g) O incumprimento do disposto na alínea anterior terá por consequência a anulação da ajuda financeira;
h) A Liga dos Bombeiros Portugueses reserva-se no direito de exigir a devolução da bolsa concedida até esta data;
i) Com a conclusão dos trabalhos deverá ser apresentado um resumo que ficará na posse da Liga dos Bombeiros Portugueses, podendo esta efectuar a sua publicação segundo os seus critérios, ainda que fazendo sempre referência ao autor;
j) Os autores poderão publicar o trabalho em quaisquer outras publicações, devendo comunicá-lo previamente à Liga dos Bombeiros Portugueses.


Complete o formulário de candidatura com a seguinte documentação:
a. Declaração comprovativa da qualidade de bombeiro, inserido num dos quadros: Comando, Activo, Reserva ou Honra. Caso o candidato seja Comandante de um Corpo de Bombeiros no activo, a declaração deve ser emitida pela entidade detentora. Nos restantes casos, a declaração deverá ser emitida pelo Comandante do Corpo de Bombeiros onde o candidato se insere;
b. Curriculum Vitae do candidato;
c. Outros documentos que o candidato considere relevantes para apreciação.

Comunicação em Oncologia

Estava a ver as notícias da noite, quando apareceu uma reportagem sobre Comunicação em Oncologia.
Parece que veio cá um perito para treinar os médicos a darem as más notícias.
Às tantas entrevistam o tal perito e este diz que para dar más notícias convém não mentir, não ocultar e ser simples na mensagem.

Claro que me lembro logo da intervenção em crise e das regras que seguimos quando estamos a lidar com as pessoas no meio da situação. As mesmas regras que aplicamos, aliás as mesmas regras que deveríamos todos aplicar na vida.
